Transportation to Medical Appointments

Policy

Foster parents are responsible for providing or arranging transportation for medical and dental appointments. When transportation services through Connecticut Access health plans are utilized, foster parents should attend all appointments with their foster children (except as noted below).

Children Under Twelve (12)

Children under twelve (12) years of age shall be accompanied to all medical appointments. If the foster parent is unable to accompany the child to the appointment, one of the following, as appropriate, shall be responsible for providing an attendant for transportation:

  • child’s parent
  • foster parent
  • caretaker
  • legal guardian
  • the Department of Children and Families.
Children between Twelve (12) and Fifteen (15)

Children between the ages of twelve (12) and fifteen (15) years may be transported without an attendant if a consent form is signed by a:

  • parent
  • foster parent
  • caretaker
  • legal guardian
  • the Department of Children and Families.
Children Sixteen (16) and Older

Children sixteen (16) years and older may be transported without an attendant and no consent form is required.
