Medical Services for the Non-Committed Child

Non-Committed Children When a non-committed child is accepted for placement and supervision, form DCF-449 is used, signed in triplicate, to obtain signature(s) of the parent(s) authorizing the Department to place and provide the necessary medical, dental and hospital care and to release information to the Department. The original of the form remains in the case record and additional copies are for the doctor and hospital as needed.

The Department will pay for medical care for non-committed children only under the conditions authorized by policy. In all other cases, the worker arranges payment for expenditures with the appropriate agency, town, or parent and notifies, in writing, the person who has responsibility for the child of the financial arrangements and to whom bills are to be submitted.
