INSTRUCTIONS TO CARETAKERS

 FOR THE HEALTH CONTENT OF THE PLACEMENT PORTFOLIO

 ·         The health content of the Placement Portfolio is strictly CONFIDENTIAL and may not be copied except for the doctor’s records or the Department of Children and Families’ records.  Appropriate content in the Portfolio can be shared with the school as needed.  (For example, a diabetic child needing a mid-morning snack.) 

·         Please bring the Placement Portfolio to every medical and dental appointment for the child. 

·         Please note the following health appointments that are required for the child: 

-        an initial health screening within three (3) days of all placements 

-        a dental checkup scheduled within fourteen (14) days of the first placement and a dental exam within thirty (30) days (excluding infants) 

-        a comprehensive multi-disciplinary exam within thirty (30) days 

-        a well child exam (annual physical) within sixty (60) days.

Note:  The latter three health appointments may be deferred if DCF has included current reports in the Portfolio.  Please call me to discuss the need for these appointments.

·         To locate health providers for the child, please contact me or the local Department of Social Services. 

·         Ask the physician or his/her staff to complete the Report of Health Care Visit (DCF-742) and return it to you.  Place one copy in the Placement Portfolio and give one to DCF. 

·         Please bring the Placement Portfolio to all Administrative Case Reviews. 

·         If the child leaves your care, be sure that the Placement Portfolio accompanies the child.
