INSTRUCTIONS TO HEALTH CARE PROVIDERS

 FOR THE  HEALTH CONTENT OF THE PLACEMENT PORTFOLIO 

The HEALTH CONTENT in the Placement Portfolio is utilized to: 

·         provide better continuity of care for children in out-of-home placement 

·         ease the task of the health care provider in obtaining and transmitting medical data 

·         improve communication between providers and the Department of Children and Families. 

The Placement Portfolio will be kept by the caretaker who will be responsible for bringing it to every health care visit.  Do not hesitate to photocopy the health contents for your medical record. 

If this is the child’s first visit to you, please review the Portfolio and note any important or confusing data you may wish to clarify with DCF. 

Please complete the “Report of Health Care Visit” (DCF-742) and return it to the caretaker.  If you wish to speak to a DCF Social Worker, please indicate so on this form. 

In addition to the visit charge, you may bill $10.40 one time per year for completing the “Report of Health Care Visit” (DCF-742) form using Code 99080 on the usual H.C.F.A. 1500 Form to the Department of Social Services.
