Placement Portfolio: Medical Alert

Completion and Delivery

The Social Worker shall include a Medical Alert (DCF-741) in the Child’s Placement Portfolio to provide health information to the caretakers that will immediately alert them and the health care provider of any potential health concerns for the child. 

For example, allergic reactions to bee stings, medications, food, or environmental factors should be immediately added to the DCF-741.

Completion and Delivery

The DCF Social Worker shall 

·          complete the Medical Alert (DCF-741) in LINK with available information every time a child enters a new placement 

·          provide a copy of the DCF-741 to the caretaker immediately at the time of placement as part of the Placement Portfolio. (See 44-5-6.1) 

·          review the information on the DCF-741 with the caretaker and answer any questions. 

A caretaker is defined here as a person responsible for a child’s care such as the child’s guardian, foster/relative/adoptive parent, or an employee of a public or private residential home or facility or other person legally responsible under state law for the child’s welfare in a residential setting.

Updates and Reviews

The Social Worker shall 

·          be responsible for all updates to the DCF-741, when necessary 

·          if there are any significant changes, provide an updated copy of the DCF-741 to the caretaker to be filed in the Placement Portfolio 

·          review the DCF-741 every six (6) months at the time the Treatment Plan is completed.
