Placement Portfolio: Health History

Policy

The Social Worker shall document the child’s health history on DCF-469, “Child Placement Information”, and include it in the Placement Portfolio which is provided to the caretaker at the time of placement. 

The child’s Social Worker shall 

·          complete the health history as fully as possible.  Information that is not available at the time of placement shall be provided to the caretaker within thirty (30) days of the date of placement. 

·          update the health history as new information becomes available.

Who Collects Information When a Case is Transferred

In cases that are transferred from an investigator to an assessment worker, the process of collecting the information shall begin during the investigation and carry over to the assessment phase in a collaborative effort by both workers. 

Both Social Workers shall document in LINK the steps that were taken to secure the necessary information.  For example, items to note may include letters sent to previous health providers, telephone calls, other applicable actions.

Sources of Information Sources that may be used to complete the health history include the parents, relatives, health providers, school health records, case record information and medical data via LINK.
Updates and Reviews

The Social Worker shall 

·          review and update the health history on the DCF-469 at the following times:

-             after any significant event

-             every six (6) months at the time the Treatment Plan is reviewed, and 

·          provide the updated DCF-469 to the caretaker to be filed in the Placement Portfolio.
