Placement Portfolio: Caretaker Log of Visits to Providers

Policy

Each caretaker shall be given a supply of the “Caretaker Log of Visits to Providers” (DCF-2127) as part of the child’s Placement Portfolio. 

The caretaker will complete the log for each visit to a health care provider. 

The log is not filed in the Uniform Case Record, but rather, is a record to be maintained and retained by the caretaker in the Placement Portfolio.

Purpose of Log

The purpose of the log is to help the caretaker to track the provider visits and give a general overview of the health care provided the child while in placement, even if there is no corresponding “Report of Health Care Visit” (DCF-742) which has been completed by the health care provider. 

The log would be especially helpful in the case of a series of counseling sessions where the provider only filled out an initial “Report of Health Care Visit” (DCF-742).

Utilizing the Log

When visiting the child, the Social Worker shall review the DCF-2127 log with the caretaker to establish if all applicable “Report of Health Care Visit” forms (DCF-742) have been returned. 

If a needed form is missing, the Social Worker shall contact the health care provider to obtain the provider’s record of the visit.
