Case Transfer Conference

Policy When the investigator and supervisor decide to transfer a case to a DCF service unit, a case transfer conference shall always be held for cases assessed as high risk.

A case transfer conference may be held for cases other than high risk as deemed necessary by the worker and supervisor.

Time Frame for Conference When required, a case transfer conference shall be held within five (5) working days of the case being assigned to the Social Worker in the service unit.
Coordination of a Transfer Conference The investigator shall be responsible for the coordination of the case transfer conference and for ensuring that written notification is sent to the participants, using LINK forms, "Notification to Family of a Case Transfer Conference" (DCF-2087) and "Notification to Provider of a Case Transfer and Case Transfer Conference" (DCF-2088).
Persons Who Must Attend a Case Transfer Conference Those who must attend a case transfer conference are
  • the investigator
  • the investigation supervisor
  • the supervisor of the service unit
  • the receiving worker, if the case is assigned
  • a representative of a community service agency providing services to the child and the child's parents

Any member of the Regional Resource Group who participated in the investigation may attend.

Persons Who Shall Be Invited to Attend The following persons shall receive a written invitation to attend the case transfer conference:
  • the parent(s), adolescent child, and attorneys for the parent and/or child
  • any professional (e.g., medical provider, psychologist, community consultant, etc.) who participated in any aspect of the investigation.

Every effort shall be made to hold the transfer conference at a time that the parent is able to attend.

If the parent is not able to attend due to lack of transportation or the need for child care, the DCF worker shall offer assistance with transportation and, where possible, child care.

Notification When a Case Transfer Conference Is Not Necessary If a transfer conference is not necessary, the supervisor of the service unit shall send written notification of the transfer, including the new Social Worker's name, by use of Notification to Family of Case Transfer (DCF-2089) to the following persons:
  • the parents
  • the adolescent child
  • attorneys for the parents and child.

Providers shall be notified by use of Notification to Provider of a Case Transfer and Case Transfer Conference (DCF-2088).
