Treatment Plan Monitoring

Monitoring of the Health Treatment Plan

Policy The Treatment Worker is responsible for monitoring the implementation of the Health Treatment Plan through visits and telephone contacts with the birth parent(s), relatives, foster parent, or other caretakers, and the professionals involved in the child's health care.
Procedure The Treatment Worker shall:

Ÿ follow the schedule of contacts as outlined in this section (36-15-1)

Ÿ maintain schedule of contacts with professionals for the purpose of determining that appropriate activities are occurring and that the plan is fulfilled according to schedule

Ÿ collect the required monthly written reports provided by professionals

Ÿ include documentation as required in this section (36-15-1) and maintain ongoing case contact logs (36-15-4).

Note: The health contact event shall be recorded on Case Activity Notes (DCF-2024) and the substantive recording of the event shall be on Case Activity Notes-Medical (DCF-2026).

Child Receiving Mental Health Services For the child receiving mental health services, the worker shall provide for case review through contact with the child's mental health therapist:

Ÿ once each month for a child in a foster care home, and

Ÿ twice each month for a child in an acute care psychiatric facility or long term residential care setting.

This review shall include the child's symptoms, treatment objectives, and plans for less restrictive placements.

Amendments and Problem Solving All reasonable efforts shall be made to resolve any problem identified through required monitoring using procedures outlined in this section (36-15-5).

If identified problems cannot be ameliorated, the worker with his/her supervisor and any professional whose assistance is needed, shall determine and document in writing whether to continue or amend the child's health treatment plan.

Recording All information that is compiled as a result of monitoring shall be filed in the child's uniform case record.
