Trainee’s Training Record

Policy

Training records of Department trainees shall be maintained in a consistent manner by all Regional Training Unit Supervisors, pursuant to the requirements of this policy.

Contents of Records

All trainee training records shall contain

  • the date each basic curriculum was completed
  • the date cases were assigned after completion of each curriculum
  • the total number of open cases assigned to date
  • a caseload listing
  • issues and concerns the trainee needs to address
  • the date cases were transferred or closed
  • a copy of the three (3) month evaluation completed on the Performance Appraisal form (PERS 125) if any "Job Elements" on the form were rated "Less than good"
  • a copy of the six (6) month evaluation completed on the Performance Appraisal form.
Regional Training Unit Supervisor Responsibilities

Regional Training Unit Supervisors shall

  • record activities and assignments in LINK
  • note in the third month evaluation any issues or concerns, which may require further training, in the training record
  • immediately forward a copy of the third month evaluation to the Training Academy if any issues or concerns, which may required further training, are identified in the evaluation
  • ensure that a copy of the six month evaluation be incorporated into the training record.
Storage of Training Records

Following the completion of a Regional Training Unit assignment, the trainee’s record shall be placed in the trainee’s regional office personnel file.
