Adoption Plan

Preparation of the Adoption Plan In preparing the adoption plan, the adoption specialists shall:

•meet with the treatment worker

•review the Uniform Case Record

•meet the child and the caregivers

•complete the assessment, and

•write the adoption plan narrative.

The adoption specialists shall then fill out the Adoption Plan Summary (DCF-336A) which includes the dates for the adoption plan narrative, Checklist for Recruitment Plan (DCF-2038), Child Assessment (DCF-2039) and Adoption Readiness Case Consultation (DCF-2040). The Adoption Plan Summary form shall also include factors that will influence the selection of an adoptive family such as:

•a description of the special needs of the child

•the child's need to be placed in a family of like racial, cultural and/or language background, and

•medical, psychological and/or educational, behavioral needs or necessary support services.

 

Other Needs Included on the Plan The plan shall also include the child's need to be placed in a family of like racial, cultural and/or language background, and medical, psychological and/or educational, behavioral needs that will influence the selection of an adoptive family. (This will also be used to present to the Permanency Planning Team.)
