Adoption Placement Process

CMS Coding at Placement

Case Establishment Form At the time of placement into the adoptive family, the child's name may be changed to include the adoptive family's name. On the Case Establishment - Referral Form (DCF-379), change the name of the child and list under also known as (AKA) the child's birth name. Do not change the number. If this is a foster parent adoption, DCF-379 is completed after the Agreement for Placement in Adoption (DCF-421) is signed.

The Data Entry Operator must enter the DCF-379 before other forms with the new name. By maintaining the child's CMS number, with a new name and address change, this eliminates the need for a new treatment plan.

Note: The Data Entry Operator may need to page through various CMS and VPS screens to change the child's name. The child will now have the adoptive name with their old number.

Case Treatment Plan Control Form Case Activity Form (DCF-386) will remain in effect; the placement will continue to be coded as a foster care placement until the child's adoption is finalized.
Medical Number The child's birth name should be listed under AKA on DCF-379. This will not affect the child's medical number.)

The eligibility specialist or regional staff person will request a new EMS number of the child going into adoptive placement by completing a W1Y (if the child is not Title XIX eligible) or a Title IV-E/Title XIX Application (DCF-550) (if the child is Title XIX eligible) with an enclosed memo to the Chief of Eligibility Services with the Department of Social Services.

If the child is IV-E eligible, only the initial date on the DCF-550 and questions #10, #16, and the declaration of citizenship, is completed. The child will then be given a new EMS number.

Placement and Reimbursement/Payment Form Placement and Reimbursement/Payment Form (DCF-383) should be completed and coded subsidized adoption (03).
At Finalization At the time of finalization, the child's name should be changed back to the birth name in DCF-379 and the case is then closed. If this is to be a subsidized adoption, the Adoption Subsidy Agreement, Initial Agreement (DCF-418-I) may use the child's adoptive name.
