| When Eligibility Begins |
Eligibility for Title IV-E Adoption Assistance begins at the
time of the adoption finalization. |
| Monthly Subsidy |
Only regular monthly financial payment subsidies are Title
IV-E eligible. Lump sum payments may qualify if they do not exceed the subsidy maximum. |
| Annual Review |
Annual review of the continuing need for and the amount of
subsidy is necessary. Unlike the Title IV-E Foster Care Maintenance Payments program, a
redetermination of technical eligibility factors as they relate to the AFDC program is not
required. An annual review is accomplished by having the adoptive family complete Adoption
Subsidy Agreement Annual Agreement (DCF-418-A) and Departmental staff complete the Title
IV-E/Title XIX Redetermination Form, Adoption Assistance Program (DCF-552A). |
| When Eligibility Terminates |
Title IV-E eligibility terminates when any one of the
following occur:
The child reaches the age of eighteen (18).
The parents are no longer legally responsible for the support of the child (i.e.,
military service, emancipation).
It is determined that the child is no longer receiving any support from the
adoptive parents.

| Parents Requirements |
Parents are required to notify the Department of changes in
circumstances that would make them ineligible for adoption assistance payments or eligible
for payments in a different amount. |
| Eligibility Forms |
The eligibility specialist must complete Title IV-E Adoption
Subsidy Application (DCF-552) for all children at the time of the initiation of a monthly
financial adoption subsidy payment. In addition, the Title IV-E eligibility status must be
entered on CMS. Termination of the subsidy and the resulting Title IV-E ineligibility must
be entered in CMS. |
