Procedures for Name Change at Time of Placement

Procedures The procedures for changing the child's name in the CMS System and with the Department of Social Services are as follows.
Changing the Name At the time of adoptive placement, Case Establishment-Referral Form (DCF-379) is completed by the Social Worker, changing the child's name to the adoptive family's name, or if this is a foster parent adoption, DCF-379 is completed after the Agreement For Placement Before Adoption (DCF-421) has been signed.

The child's birth name should be listed under AKA on DCF-379. (This will not affect the child's medical number).

Note: The Data Entry Operator (DEO) must enter the Case Establishment-Referral Form (DCF-379) before other forms with the new name.

Notification of Name Change The eligibility specialist must be notified of the name change.
Address Change The address should also be changed if necessary.
CMS Number Maintained The CMS number will be maintained. By maintaining the child's CMS number, with a name and address change, this eliminates the need for a new treatment plan and review.

Note: The DEO may need to page through various CMS and VPS screens to change the child's name.

New EMS Number The eligibility specialist or regional staff will request a new EMS number for the child going into adoptive placement by completing a DCF-550 with an enclosed memo to the Chief of Eligibility Services with the Department of Social Services' Central Office. Only the initial data, on DCF-550 and questions #10, #16-27 (as they pertain to the child only) and the declaration of citizenship are completed.
Committed Child Coding Case Activity Form (DCF-386) will remain in effect and will continue to be coded as committed child (2) until the child's adoption is finalized.
