Procedures to Place the Special Needs Child In Adoption

Requirement Parental rights have been terminated and the child cannot and should not return home.
Special Needs Designation The child has been designated a special needs child according to all the appropriate certification criteria.
Certification on Form DCF-416 The certification of the child as special needs and as eligible for subsidy will be recommended by the Program Supervisor based on the criteria for "Special Needs" submitted by the Social Worker and Social Work Supervisor or the private Connecticut adoption agency staff on the Certification of Special Needs Status (DCF-416).
Program Supervisor Certification The Program Supervisor reviews all the written material submitted by the Social Worker, Social Work Supervisor, or the licensed private Connecticut child placing agency and, on the basis of this information, certifies the child as a "special needs" child, eligible for the Subsidized Adoption Program.
