Processing Subsidized Adoptions

Introduction This topic covers

•the forms that are required to finalize a subsidized adoption, and

•how the forms must be processed.

These forms, called "subsidy forms," are in addition to those required for completing a Department adoption.

Subsidy Forms The table below lists the subsidy forms the Social Worker must complete for a subsidized adoption.

DCF-416 Certification of Special Needs Status (one in child's birth name and one in child's adoptive name) To complete and sign the form, and give it to the Social Work Supervisor who will submit it to the Program Supervisor for approval.

(Initial form)

Adoption Subsidy Agreement, Initial Agreement (use child's adoptive name) To complete the form in duplicate, and

• sign the form as the Department representative, and

• obtain the signature(s) of the adoptive parent(s), and

• return one copy to adoptive parents

DCF-419 Commissioner's Statement of Support (one in child's birth name and one in child's adoptive name) To complete the form in duplicate for the Commissioner's signature.
DCF-739 Application For Reimbursement For Non-Recurring Adoption Expenses (use child's adoptive name) Notify adopting parents they may apply; complete Section I, II, and III; and forward to adopting parents for completion and signature.
DCF-738 Agreement For Reimbursement Of Non-Recurring Adoption Expenses (use child's adoptive name) To complete, sign and forward to adopting parents for signature. Then forward to Program Supervisor for approval and signature.

Note: The entire packet is prepared by the Social Worker for the Social Work Supervisor's and Program Supervisor's approval.

 

Procedure:

Pre-Court

The table below lists the steps the Social Worker must complete to submit a subsidized adoption to the Probate Court.

 

Step Action
1 Complete and process the Probate Court forms, the Department subsidy forms listed in the previous table, and the Probate Court Study for the child being adopted.

Cross-reference: See policy 48-19-1 to 48-19-7 of this chapter.

Note: Include the words "Subsidized Adoption" at the top of the Probate Court Study. This information is important, because a subsidized adoption incurs no court fees, per CONN. GEN. STAT. §45a-111(b).

2 Forward the Probate Court Study and the forms indicated in Step 1 to the Adoption Resource Exchange.

Note: The Adoption Resource Exchange returns the Probate Court Study and the forms to the regional office, when they have been signed by the Commissioner.

3 Prepare a cover letter for the Probate Court that indicates the proposed adoption is subsidized, with the words "Subsidized Adoption" in the heading "RE:" at the top of the cover letter.
4 Submit the cover letter, the Probate Court Study, and the adoption forms to the Probate Court.
5 Submit Title IV-E Adoption Subsidy Application form (DCF-552) to the eligibility specialist for completion and determination when the adoption packet is sent to the Probate Court.1
Procedure: Post-Court  

The table below lists the steps the Social Worker must complete after the adoption has been finalized by the court.

 

Step Action
1 On the date of the adoption finalization, close the child's record by completing the necessary CMS forms.
2 Complete Adoption Report (DCF-420).
3 Initiate procedures to request that the case information be expunged from CMS.
4 Open a new subsidized adoption case on the CMS system, when the adoption has been legally finalized.

Note: Use the child's new name and new CMS number.

5 Do the foster/adoptive parents already have an active payee number in the CMS system?

• If yes, use their existing payee number.

• If no, apply for a new payee number for them.

6 Collect all completed forms needed for the subsidized adoption record. (See "Subsidized Adoption Record" on the following page.)
7 The regional office transfers the subsidy case record to the Adoption Resource Exchange, where the record will be reviewed. If it is determined the case is appropriate for transfer, Worker Change/Assignment Form (DCF-382) will be completed by ARE.
8 The subsidized adoption record should be sent to the Adoption Resource Exchange within forty-five (45) days of finalization.

Note: The Adoption Resource Exchange requires these papers because it is responsible for completing the annual redetermination of all adoption subsidy cases.

The closed adoption record must accompany the subsidized adoption case record. The Adoption Resource Exchange office will forward the closed record to the Processing Technician for recording and storage.

Copies of court studies, dictation, etc. should NOT be a part of the subsidy record. See Subsidized Adoption Face Sheet (DCF-2051) for required forms.

Note: Only the child's new adoptive name should be used on all forms in the subsidy record.

   
Subsidized Adoption Record The Social Worker must make sure that each subsidized adoption record contains completed copies of the following forms:

•Certificate of Special Needs Status (DCF-416)

•Adoption Subsidy Agreement Initial Agreement (DCF-418-I)

•Commissioner's Statement of Support (DCF-419)

•Subsidized Adoption Face Sheet (DCF-2051)

•Title IV-E/Title XIX Application (DCF-550)

•Title IV-E Adoption Subsidy Application (DCF-552).

Note: In completing the form DCF-416, the sections "previous attempt to find other resources" or "has established significant emotional ties with foster parents" must be checked if the child is eligible for Title IV-E. Also, form DCF-550 should be the initial form completed when the child was most recently placed in foster care.

Forms for a New Subsidy Case The Social Worker must complete the following forms to open a new subsidy case:
  • Demographic Form (DCF-375)
  • Case Member Form (DCF-378)

  • Case Establishment - Referral (DCF-379)

  • Placement and Reimbursement/Payment Form (DCF-383)

  • Case Treatment Plan Control Form (DCF-386)

Note: Eligibility Form (DCF-374) is submitted by the Eligibility Specialist.

Connecticut Child Placing Agency May Request Subsidy A Connecticut licensed child placing agency may request an adoption subsidy for a child for whom they are statutory parent. The licensed agency shall apply to the Department regional office which covers the town where the adoptive parent reside.

The licensed private agency must complete, in duplicate, DCF-416, DCF-418-I, DCF-419, DCF-738, DCF-739 as well as a copy of the Order of Termination of Parental Rights and a copy of the Probate Court study and submit to the Program Supervisor who has responsibility for Permanency Planning or Adoption Units.

 

The Program Supervisor will sign DCF-416 and determine the financial subsidy rate and sign DCF-418-I. This request will be submitted for the Commissioner's approval and signature through the Adoption Resource Exchange. The Program Supervisor will return one copy of the forms to the private agency. It is the responsibility of the Connecticut private aency to notify the Department by submitting an Adoption Decree, so a subsidized adoption case may be opened effective the date of finalization.
