Adoption Specialist

Job Functions Adoption specialists shall be specially trained Social Workers involved in permanency planning, facilitation of adoption, and provision of services related thereto for children and prospective adoptive parents.
Services Adoption specialists may provide the following services:

•consulting with the treatment workers regarding all aspects of adoption

•participating in assessment, or securing information for the assessment of the child's readiness to move into adoptive placement

•at the request of a child's Social Worker, developing a written recruitment plan for a child when the child's special needs will require intensive, on-going placement efforts

•developing written adoption plans for children on the specialist's caseload

•when appropriate, preparing a case for Termination of Parental Rights in Superior Court or preparing a case for Voluntary Consent in the Probate Court

•assisting in the development of standard forms to document long term care agreements

•providing consultation and/or training to the treatment worker or foster parents regarding development of Life Books

•working with age appropriate children in the Life Book development process

•preparing children for adoptive placements

•preparing foster parents for adoption or release of the child placed in their home

•preparing prospective adoptive parents for adoption of the child who will be placed in their home

•arranging pre-placement visits

•arranging for and monitoring receipt of services through the Permanency Placement Services Program (PPSP)

•offer and then provide post-adoption services for a six (6) month period.
