Legal Risk Information Protocol

Information for Prospective Adoptive Parent(s) The following information shall be provided verbally and in writing to assist the prospective adoptive parent(s) in making an informed decision about taking the child(ren). This information will allow the parent(s) to best meet each child's individual needs, if placement occurs. Such information shall include but not be limited to:

•Genetic Parent(s) Information (DCF-337) and Medical Information On Genetic Parent(s) (DCF-338).

•All known medical and mental health information, including the Medical Passport (CF-741), copies of developmental records and shared content of psychological, psychiatric and neurological reports when appropriate.

•Child Placement Information (CYS-818).

•Information regarding all aspects of the child's background including significant events and placement history.

•Life Book.

•Statement of legal risk, outlining current detailed legal status including any expectations of the family and the Department.
