Notice by Certified Mail
| Policy |
The person coordinating Service of Process shall
use the following procedures to prepare and serve documents by certified
mail. |
| Green
Card Procedures |
These procedures shall be followed for completing
the green card.

| What Is
To Be Served? |
The following
documents must be served by a Judicial Marshall:

In box # 3, fill in the name and address of the
client, Judicial Marshal or newspaper, depending on the action required. |

Note: An overnight carrier may be used only in the event overnight
delivery is necessary for service to be made on time.

3 |
On the front, bottom, left-hand corner of the
green card, place the childs name (first initial and last name) and hearing
date for case referencing; for example, K. Doe, 1/15/97. |

4 |
Check
appropriate service type boxes:
return
receipt
and certified for mailing to newspapers and Judicial Marshal
restricted
delivery, addressee only
for clients being served via certified mail (mailed by a Judicial
Marshal); e.g., out-of-state parent(s) with a known address. |

Mail Receipt Procedures |
Follow these procedures to complete the receipt:
| 1 |
Fill in the address of the party to receive the
documents. |
| 2 |
If the Judicial Marshal is providing certified
mail service, do not attach the receipt to the envelope.
The Judicial Marshal will do this when he/she mails it. |
| 3 |
Do not fill in the postage amount.
The Judicial Marshal will do this. |

| Envelope
Procedures |
Follow these procedures for the envelope:
| 1 |
For
documents being mailed to Judicial Marshal or newspapers,
use an envelope with the Department area office address on it.
For
documents being served on clients,
place in a plain envelope with no return address.

| 2 |
Type the name of the person to be served on the
front of the envelope. |
| 3 |
DO
NOT seal the petitions to be served inside the
envelope. The Judicial Marshal
needs to stamp the documents. The
Judicial Marshal will seal the
envelope. |
