Licensing of Special Study Foster Homes
| Policy | Upon
receipt of the familys application for licensure (DCF-354) and the preliminary
assessment (DCF-2144) completed by the childs worker, a FASU worker shall conduct a
licensing assessment to determine if the applicants can comply with foster care licensing
regulations and can meet the needs of the specific child(ren) who will be placed in the
home. This assessment
shall be completed within sixty (60) days of assignment. The applicant
must complete any pre-licensing training required by the Department. |

Assign
the case to a FASU Social Worker. ·

Within sixty (60) days of
assignment, complete the following steps: ·
Make a
home visit(s) to conduct the assessment. ·
Verify
that the family has received all forms which must be completed for licensure. ·
As necessary, assist the
family to complete licensing forms. ·
Enter
appropriate data regarding the family in LINK. ·
Document
the assessment on the following forms: -
DCF-805, Assessment
for Licensure for a Relative, Special Study, or Independent (Interstate) Home -
DCF-0043, Verification
of Requirements for Foster Care or Adoption Licensure -
any other supporting
documentation for licensure. ·
Submit
the completed licensing record to the FASU Supervisor and Program Supervisor for approval. |
