Referral and Placement Process

Policy The regional staff shall identify those children who may meet the criteria for this program.
Steps in Referral and Placement Process The following are the steps in the referral and placement process:

1 DCF Social Worker and Social Work Supervisor
  • Decide to seek a placement in Therapeutic Foster Care, typically based upon knowledge of the case and recommendation from a clinician(s).
2 DCF Social Worker
  • Complete the referral request, using DCF-469, "Request for Foster Home Placement".
  • Submit the referral request with cover memo and appropriate materials to the regional liaison.

Note: In some regions, the request may be submitted to the Review Team before being forwarded to the liaison.

3 Regional Liaison
  • Share the referral request(s) with the Regional Placement Review Team.
4 Regional Placement Review Team
  • Decide to

- proceed with the referral, or

- deny the request, or

- approve another type of service.

5 Regional Liaison
  • Telephone the referral to the contracted provider agency, and
  • Send a copy of the cover letter and DCF-469 and other appropriate materials to the agency.
6 Provider Agency
  • Telephone the liaison with an immediate response if no families are available, and
  • Send written confirmation to DCF within three (3) days.

or

  • Request more information, and/or
  • Arrange to read the DCF case record and, as appropriate,
  • Visit with the child and former caretakers.
7 DCF Social Worker
  • Send additional information as requested by the agency.
8 Provider Agency
  • Decide to accept or reject referral within ten (10) working days of receiving the information about the child, and
  • Provide DCF with written confirmation of acceptance, wait-listing, or rejection within five (5) days of decision.
9 DCF Social Worker
  • Update the uniform case record and LINK with placement information.
