Case Review Process

Purpose of Review The purpose of the case review conducted by the SRU is to
  • review each case defined in DCF Policy 34-25-2
  • review the appropriateness of the Department’s services to the child and family
  • identify instances of exceptional service provision
  • identify service provision that requires corrective action
  • where indicated, identify needed improvements to Department statutes, regulations, policies, procedures, practices, training, performance and use of collateral systems.

Process
The table below presents the SRU case review process:

 

Person Responsible Activity
SRU Reviewer Within thirty (30) days of receipt and assignment of a case, conduct and complete a review to accomplish the purposes stated above.

Include the following activities in the review:

  • Confer with the Regional Administrator or facility Superintendent to obtain preliminary case information.
  • Review
    • the entire case history of the Department’s contact with the family
    • the circumstances of the child’s death
    • the case record and other pertinent recorded material; e.g., supervisory notes
    • the autopsy report, if applicable
    • the death certificate
  • Interview any DCF staff member having involvement with the case or other appropriate parties involved.

Note: DCF staff who are interviewed may have a union representative who is a DCF employee present at the interview.  When foster parents are interviewed, they may have a foster parent liaison present. The representative or liaison must sign a confidentiality statement prior to the interview.

  • Submit a draft report to the Bureau Chief of Quality Management.

  • At the conclusion of the review, submit the draft report to the
    • Commissioner
    • Deputy/Assistant Commissioners
    • Bureau Chief of Child Welfare Services
    • Regional Administrator for the involved region.
  • Distribute the final report.
  • When requested, conduct an exit interview with affected staff. (May be done by a designee.)
