Case Review Report

Written Report Within thirty (30) days of notification to the SRU of a child’s death or other incident to be reviewed, the SRU shall submit a written draft report of the case review to the
  • Bureau Chief of Quality Management
  • Commissioner
  • Deputy/Assistant Commissioners
  • Bureau Chief of Child Welfare Services
  • Regional Administrator of the involved region.
Contents of Report The report shall include
  • a chronology of the events
  • a summary of services provided to the child and family
  • a determination as to whether the Department
  • -  provided adequate and appropriate services to the child and family
    -  complied with regulations and policy

  • recommendations and guidelines for corrective action.
Distribution of Report When the report is approved by the Commissioner, the SRU shall distribute the report to involved management staff.
Exit Interview When requested, the SRU shall conduct an exit interview with involved staff.

Staff may have a union representative who is a DCF employee present during the exit interview. The representative must sign a confidentiality statement prior to the interview.

The notes from the exit interview shall be included in the SRU case report.

Confidentiality The SRU report is a confidential document that cannot be duplicated nor filed within the child’s case record.

The report shall be maintained in the SRU files.
