Response Letter to Mandated Reporters

Policy The Department shall provide mandated reporters with information regarding the acceptance and outcome of reports made by them in behalf of children and families.

Response Letter to Mandated Reporters The table below presents the procedures for sending a DCF-2122, "Response Letter to Mandated Reporters."

The letter must be sent twice:

  • at the time of report acceptance
  • at the conclusion of the investigation.

 

Person Responsible Action
Hotline Worker
  • Complete Sections (1), as needed; (2), and (7).
  • Submit the letter to the supervisor for approval.
Hotline Supervisor
  • Approve and sign the letter.
  • Ensure that the letter is sent to the mandated reporter immediately at the time of report acceptance.
Investigator
  • Complete Sections (3) - (6), as appropriate.
  • Submit the letter to the supervisor for approval.
Investigation Supervisor
  • Approve and sign the letter.
  • Ensure that the letter is sent to the mandated reporter within ten (10) working days of the completion of the investigation.
