Service Agreement/Safety Plan

Policy

A Service Agreement/Safety Plan (DCF-2166) may be used in the following situations:

  • Prior to having a formal, written treatment plan, an investigator may use the Service Agreement/Safety Plan to ensure that parents or legal guardians and the Department of Children and Families are aware of what needs to be accomplished to ensure that the children involved are safe and well cared for.

  • After a treatment plan has been written and prior to writing the next treatment plan, the Social Worker may use the Service Agreement/Safety Plan as an interim tool to make minor modifications to the Treatment Plan, e.g., to further emphasize or clarify certain steps needed to carry out the treatment plan objectives.

  • The Service Agreement/Safety Plan shall not be used to replace the formal Treatment Plan or modify Treatment Plan goals.  Any change in the goals or plan outcome requires a planning conference and the writing of an updated Treatment Plan.
