Initial Treatment Plans for Children in
Foster Care: Additional Components
| Additional Components |
For children placed in foster care, the Initial Individual
Treatment Plan (DCF-529-I) shall include the following additional components:
A description of the type of home or institution
in which the child is to be placed and why it is appropriate.
How the agency responsible for the child plans to
carry out the voluntary placement agreement or judicial determination made with respect to
the child.
A discussion of the appropriateness of the
services provided to the child.
Provisions to ensure that while the child is in
out-of-home care, he/she receives proper care and has his/her individualized needs met.
Provisions for needed service to the child's
foster parent in order to maintain placement and meet the child's needs.
A schedule and a plan for parent and child visits
including:
- frequency and duration of visits
- location
- special contingencies, and
- a described procedure for amending the plan to accommodate incremental increases in
the frequency and duration of visits, including overnight visits before the child is
returned home.
